Program Coordinator
Program Coordinators at Evergreen are highly skilled individuals selected for the ability to balance and coordinate a wide variety of responsibilities, obligations, client support, advocacy, staff support, and family support. Every aspect of a client’s life is the purview of the Program Coordinator. Strong communication skills are a must as is the ability to effectively manage your own time as well as the time of clients and staff alike.
It will be your job to supervise all aspects of participant goals and programming, coordination of medical needs, and personal as well as household management. Close communication with families is required and should remain respectful and supportive at all times.
Active participation in the development of plans and routines is expected. Program Coordinators at Evergreen are team leaders and will be expected to work closely with staff members to ensure that everyone remains focused on enhancing independence and quality of life for participants.
This is a middle-management position requiring the ability to supervise employees as well as taking direction from the agency Co-directors. Your leadership skills will include the ability to guide and mentor employees.
You must be a self-starter, who is willing to accept and delegate responsibility. A Program Coordinator at Evergreen is a person who has a very high energy level, excellent interpersonal skills, a deep well of self-confidence, empathy, the ability to handle conflict, flexibility and the willingness to embrace change.
You must be able to pass a Washington State background check, lift 50 pounds and be insurable on the company auto insurance policy. Required certifications are: NAR, CPR, 1st Aid, 7 hr. HIV/AIDS class, and a high school diploma or G.E.D.
Your job will include but is not limited to:
Supervision of staff and Coordination of the medical needs for the clients on your team including but not to limited the following:
- Scheduling appointments
- Taking the client to the appointment or delegating a responsible staff person to do it for you
- Arranging the appropriate paperwork and records before the appointment
- Maintenance of all paperwork after the appointment including but not limited to nurse delegation requirements, medical books, pharmacy, and staff communications.
- Ensuring that staff (before working with the client), the pharmacy and the delegating nurse are alerted to any medical changes
Supervision and Coordination of the IISP goals for the clients on your team. You will help clients set their goals and you will ensure that goals are being met in a timely manner including but not limited to the following:
- Scheduling outings, appointments, and transportation
- Assisting the Director (s) in design and implementation of training routines and strategies.
- Monitoring, scheduling and delegation of exercise goals> Will assist the administrative coordinator in monitoring the record keeping associated with client goals.
- Observing the efficacy and relevance of goals in place
- A monthly review of the client’s IISP goals as well as their social life, their finances, and any other pertinent information will be completed and submitted in writing to the Co-directors, and the client’s family
Supervision and Coordination of client’s Home, Health, and Safety needs including but not limited to the following:
- Fingernail care, Oral care, Bathing, and overall Hygiene maintenance
- Grocery shopping and banking
- Budget tracking and planning with the cooperation of the Directors (s)
- Paying bills with your clients
- Meal and menu planning individual to the participant. Meal and menu planning will be done with the participant’s doctor and/or nutritionist only in the case of health and/or safety issues.
- Fire drills and/or fire safety training will be performed no less than once monthly and a safety checklist will be completed monthly.
- Apartment maintenance and cleaning (daily, weekly, monthly, yearly)
Supervision and Coordination of employment goals for the participants on your list including but not limited to the following:
- Ensuring that transportation to and from work is arranged.
- Communicating with the vocational provider and or the em
- Assisting the client and the vocational provider to determine employment goals when applicable
- Establishing routines with clients and supervising staff members to ensure that participants arrive at work on time, looking and feeling their best
Encouraging and supporting healthy family relationships including but not limited to the following:
- Once weekly contact with at least one family member. Once monthly contact with each family member unless you are specifically instructed not to. Documentation and record keeping of the contact made and the outcome
- Keeping track of family birthdays and special events and ensuring that family members receive cards/presents from your clients
- Assisting the client in developing healthy relationships. Outings and activities will be selected using methodology that incorporates client preference and w/an eye towards building new or strengthening old relationships.
- Assist the administrative coordinator in maintaining your client’s program books
Supervision and Coordination of social and recreational needs for each of the participants on your list including but not limited to the following:
- Working with the client, the client’s family and friends to determine the types recreation and social needs
- Using available resources to determine what opportunities are available, appropriate, and affordable for each client you work with
- Delegating staff members to support the client in a variety of activities throughout the week
- Arranging for or delegating all aspects of activities including issues like transportation, money, equipment etc.
- Helping the participant to budget the monies necessary for recreational activities including vacations, day trips, and smaller less expensive outings.
